Oat, Almond, Soy, or Dairy: Which Milk Should Actually Be in Your Fridge?

The milk section used to be a question about fat percentage. Now it is a wall: whole, 2%, skim, lactose-free, soy, almond, oat, cashew, pea, rice, coconut, flax, hemp, and a barista edition of most of them. Every carton is making a claim. Several of the claims are true. At least one of them is only true if you shake the carton first.

There is no single winner in this aisle, and anyone who declares one is skipping the part where different people need different things. But there is a right answer for you, and it comes down to four questions: what you are using it for, whether you need the protein, who else in the house is drinking it, and how your body handles dairy.

Here is what the evidence actually says, claim by claim, and then a decision guide you can use standing in front of the cooler.

The difference that matters most is protein

Almost everything else in this comparison is negotiable. Protein is the one place where the gap between products is enormous and fortification cannot close it.

Per 8-ounce cup, using USDA figures for dairy and the Harvard Nutrition Source comparison for the rest:

  • Cow's milk — about 8 g of protein, whatever the fat level (whole 7.86 g, 2% 8.05 g, skim 8.26 g)
  • Soy milk — about 6 to 7 g
  • Pea milk — about 8 g
  • Oat milk — 1 to 4 g depending on the brand
  • Almond milk — about 1 g
  • Rice and coconut milk — under 1 g

That spread is not theoretical. Researchers at the University of Minnesota analyzed 233 plant-based milk products from 23 manufacturers and presented the results at NUTRITION 2023. The median protein content was 2.0 g per 240 mL, with a range from 0 to 12 g. Only 16% of products matched or beat cow's milk on protein. And when they looked at calcium, vitamin D, and protein together, only 12% of products were comparable to or better than dairy on all three.

Protein quality widens the gap. A 2022 analysis in Frontiers in Nutrition compiled digestibility scores (DIAAS, which measures how much usable amino acid you actually absorb): milk protein concentrate scored 1.18, soy protein isolate 0.94 to 0.97, cooked rolled oats 0.54, almonds 0.40, and rice protein concentrate 0.37. So oat milk's couple of grams of protein are not the equivalent of a couple of grams of dairy protein. They are worth less.

This is why the FDA notes that only fortified soy beverages meet the standards to count in the dairy group, and why in February 2023 the agency issued draft guidance asking plant-based products labeled as milk to carry a voluntary statement explaining how they differ nutritionally from dairy.

None of that makes almond milk bad. If you are pouring three tablespoons into coffee, its protein content is irrelevant either way. It matters when the milk is doing a job — a glass with breakfast, a bowl of cereal, a smoothie base, a growing child's main beverage.

Shake the carton. Seriously.

This is the single most useful thing in this article, and almost nobody knows it.

Plant milks do not naturally contain much calcium. Manufacturers add it, usually as calcium carbonate or tricalcium phosphate — mineral particles heavier than the liquid around them. Over days in the refrigerator, they sink.

The Frontiers in Nutrition team measured exactly this after seven days of cold storage. Cow's milk and soy samples held their protein within 5% whether shaken or not, but the other plant beverages came out 24 to 66% lower in protein in the unshaken pour. Calcium was worse: "The unshaken almond and soy samples had 14 and 18% lower calcium contents than their well-mixed samples, whereas the unshaken rice and oat samples had 96 and 97% lower calcium contents."

Ninety-seven percent. If you pour oat milk from a carton that has been sitting untouched, most of the calcium on the label is still in the carton, as sediment, on its way to the drain.

The same paper cites earlier work by Heaney and Rafferty on eight fortified soy beverages, which averaged just 31% of the labeled calcium when unshaken, rising to 59% after shaking — the rest sitting as solid residue at the bottom of the container.

The fix costs two seconds. Shake it hard, every time, before every pour.

Read the sugar line, not the front of the box

The word to look for is unsweetened. The Harvard Nutrition Source is blunt about this: products labeled original will include added sugars. Vanilla and chocolate versions carry considerably more.

Oat milk is a genuine special case, and it is worth understanding rather than fearing. Making oat milk involves enzymes that break oat starch down into smaller sugars — that is what gives oat milk its body and mild sweetness without anything being poured in. As Harvard puts it, "Some sugars are created in the processing of oats to make oat milk, which are listed as added sugars even if no other sweeteners are added." An unsweetened oat milk can still show sugar on the label, and the label is not lying to you.

Whether that matters depends entirely on how much you drink. A splash in coffee is a rounding error. A tall glass with breakfast every day is a different conversation, especially if you are managing blood sugar. Oat milk behaves more like a light carbohydrate than like a protein food, so it does better alongside something with protein and fat than it does on its own.

For context, cow's milk shows about 12 g of sugar per cup, but that is lactose, which occurs naturally and is not counted as added sugar. Half of lactose is galactose, which the liver processes rather than sending straight into your bloodstream.

Two scary claims that do not survive the research

Soy will not mess with your hormones

This one refuses to die, and the evidence against it is about as clean as nutrition evidence gets. A 2021 meta-analysis in Reproductive Toxicology pooled 41 clinical studies published between 2010 and 2020, covering 1,753 men for total testosterone, 752 for free testosterone, 1,000 for estradiol, 239 for estrone, and 967 for sex hormone binding globulin. The finding: no significant effects of soy protein or isoflavone intake on any of them, and no effect when the data were broken out by dose or by how long men consumed it.

Soy milk also holds up well on heart markers. A 2024 systematic review in BMC Medicine pooled 17 randomized trials in 504 adults that swapped soy milk in for cow's milk, and found reductions in LDL cholesterol (-0.19 mmol/L), non-HDL cholesterol (-0.26 mmol/L), and blood pressure (-8.00 mmHg systolic, -4.74 mmHg diastolic). The authors concluded the swap does not carry the adverse effects often attributed to added sugars and ultra-processed foods.

Dairy fat is not the villain it was cast as

Whole milk does carry real saturated fat: 4.55 g per cup, against 3.07 g in 2%, 1.55 g in 1%, and 0.29 g in skim. If your saturated fat intake is already high, that is a legitimate reason to reach for a lower-fat carton.

But the leap from "contains saturated fat" to "raises your heart disease risk" has not held up for dairy specifically. A 2026 dose-response meta-analysis in Frontiers in Nutrition pooled 29 prospective cohort studies covering more than 1.6 million people. At roughly 250 to 300 g of total dairy per day, cardiovascular mortality risk came out slightly lower (HR 0.94), on a U-shaped curve where more was not better. Fermented dairy looked strongest: 200 g of yogurt per day was associated with an 11% lower risk of both cardiovascular and all-cause mortality.

The honest summary is that dairy sits somewhere between neutral and mildly protective for most people, that fermented forms look best, and that a glass of milk is not the thing standing between you and a healthy heart.

What plant milks quietly leave out

Fortification covers calcium and vitamin D in most products — 76% of oat, 69% of soy, and 66% of almond products in the Minnesota analysis carried both. What it usually does not cover is iodine.

Dairy is one of the largest iodine sources in the American diet, and the gap is not small. A 2024 review in JDS Communications reports that one cup of nonfat milk supplies about 85 micrograms of iodine — 57% of the daily requirement during pregnancy — while 30 tested plant-based alternatives averaged 3.1 micrograms per 250 mL. Plant milks do not naturally contain iodine and are typically not fortified with it. The same review notes that women of childbearing age who drink plant milks instead of cow's milk have lower urinary iodine concentrations.

Iodine matters most in pregnancy, when it is essential for fetal brain development. If you are pregnant, trying to conceive, or feeding young children and you have replaced dairy entirely, iodine is worth raising with your doctor — iodized salt, seafood, and seaweed are the main alternatives.

Potassium and zinc also tend to run lower in plant milks than in dairy, as the 2023 review Dairy and Plant-Based Milks: Implications for Nutrition and Planetary Health notes, though those are easier to cover elsewhere in a normal diet.

The gums and oils question, answered honestly

Turn a carton of oat milk around and you will usually find rapeseed or sunflower oil, gellan gum, and sometimes dipotassium phosphate. Those ingredients are what stop the beverage separating into a watery layer and a sludgy one, and what let it foam in a coffee shop. They are also what makes people uneasy.

Here is where the evidence stands. A 2021 study in Microbiome tested 20 emulsifiers against human gut bacteria in a bioreactor system. Carboxymethylcellulose, polysorbate 80, kappa carrageenan, xanthan gum, guar gum, and locust bean gum all altered bacterial density, composition, and the expression of pro-inflammatory molecules. Soy lecithin had no discernible impact. Two things are worth holding onto: this was a laboratory model using stool from a single donor, which the authors flag as a limitation, and gellan gum, the gum most commonly found in oat milk, was not among the additives tested.

The strongest human evidence concerns a different additive entirely. In a double-blind controlled-feeding trial published in Gastroenterology in 2022, 16 healthy adults ate either an emulsifier-free diet or the same diet with 15 g per day of carboxymethylcellulose for 11 days. The CMC group showed reduced gut microbiota diversity, lower short-chain fatty acids and free amino acids, and a modest increase in postprandial abdominal discomfort. That is a real signal — but 15 g a day is a deliberately heavy dose of one specific additive, not the trace of gellan gum in your latte.

A reasonable position: worth watching rather than panicking about, with dose and the specific additive both mattering a great deal. If you want to sidestep the question entirely, cow's milk and simple two-ingredient plant milks (water plus the plant, sometimes plus salt) are on most shelves, and blending your own is genuinely easy.

Where the environmental math actually lands

This part is less complicated than the internet makes it. Drawing on Poore and Nemecek's 2018 analysis of more than 38,000 farms in 119 countries, Our World in Data reports that producing cow's milk generates around three times the greenhouse gas emissions, uses about ten times the land, and consumes two to twenty times the freshwater of plant-based alternatives. The 2023 Current Environmental Health Reports review puts median per-liter emissions for soy, oat, almond, pea, spelt, and coconut milks 62 to 78% below cow's milk.

The almond water story is real but narrower than the headlines suggest. Almond milk's per-unit water use is often the highest among plant milks, particularly when weighted for water scarcity or measured per gram of protein — which matters because almonds are grown largely in drought-prone California. Even so, it uses less water than dairy.

If planetary impact is your deciding factor, oat and soy combine low emissions, low land use, and unremarkable water demand. Soy has the added advantage of being the only one that also replaces the nutrition.

The part where this gets personal

Lactose is the reason a lot of people are standing in this aisle at all. Roughly 65% of the world's adults have a reduced ability to digest lactose after infancy — but that number swings hard by ancestry, from about 5% of people of Northern European descent to 70 to 100% in East Asian communities. If dairy leaves you bloated and uncomfortable, that is not a character flaw or a moral signal. It is an enzyme.

It is also worth knowing that lactose intolerance does not require abandoning dairy. Lactose-free milk is real cow's milk with the lactase enzyme added, so it keeps the full protein, calcium, potassium, and iodine profile. Hard cheeses and yogurt are usually tolerated well, because fermentation has already digested much of the lactose.

Seeing what a swap actually does to your meals

Milk is rarely something you drink in isolation. It is in your oatmeal, your smoothie, your soup, your pancakes. Swapping oat for dairy across a week does not change one glass — it quietly changes the protein, sugar, and calcium totals in a dozen meals at once, which is nearly impossible to hold in your head.

The decision guide

For kids under 5. The 2019 consensus statement from the Academy of Nutrition and Dietetics, the American Academy of Pediatric Dentistry, the American Academy of Pediatrics, and the American Heart Association is clear: plant-based beverages are not for children under 1, and after that, cow's milk or fortified unsweetened soy milk are the recommended options. Fortified soy is the only plant milk treated as nutritionally equivalent. Almond, oat, rice, and coconut milk are fine as an ingredient in food, but they should not be a young child's main drink. The AAP's own guidance notes that alternatives generally contain less protein and fewer calories than cow's milk, and that fortification varies by brand.

If you are chasing a protein target. Dairy, fortified soy, or pea milk. Those are the three. Almond and oat cannot be made to do this job no matter how the carton is designed.

For coffee. Whatever you like the taste of — you are adding two ounces, and nutritionally it barely registers. If you want foam without a barista edition, whole dairy and soy steam most reliably, and oat is the plant milk that behaves best in espresso. Watch the label on barista formulations, which sometimes add sugar for mouthfeel.

For smoothies. Dairy, soy, or pea, because a smoothie is a meal and needs protein to keep you full. Building a smoothie on almond milk gives you a fruit drink, which is a fine thing to have and a poor breakfast.

For cereal and a glass with a meal. Dairy or fortified soy. This is the use case where the protein and micronutrient gap actually reaches your body.

If you are lactose intolerant. Try lactose-free dairy first, since it costs you nothing nutritionally. Fortified unsweetened soy is the best plant-based substitute.

If you are dairy-free by choice or allergy. Fortified unsweetened soy or pea, and pay attention to iodine from other sources.

On a budget. Dairy is almost always the cheapest protein in this aisle. In the New Zealand supermarket analysis behind the Frontiers paper, cow's milk delivered protein at roughly an eighth the cost of most plant-based beverages, with soy the closest plant competitor. The ordering is similar in American stores. If money is tight and nobody in the house reacts badly to dairy, milk is the value pick and it is not close.

If the planet is your priority. Oat or soy — and soy if you want the nutrition to come with it.

What to actually do tomorrow

Buy the version that says unsweetened. Shake the carton before every pour. Check the protein line when the milk is doing a real job in your day, and ignore it when you are just softening coffee. Keep dairy or fortified soy as the default for kids and for anyone who needs the protein. And stop treating the choice as a referendum on your values — it is a beverage, and there is more nutritional distance between two brands of oat milk than there is between most of the arguments people have about it.
