Iron, Fatigue, and Women: The Deficiency Hiding Behind 'I'm Just Tired'

You are tired in a way that sleep doesn't fix. You push through the afternoon on caffeine, your hands are cold even when the room isn't, your hair seems to be thinning in the shower drain, and lately you find yourself weirdly obsessed with crunching ice. When you mention it, the answer is some version of what you have already told yourself: you're just busy, just stressed, just getting older. Just tired.

For a lot of women, that shrug is hiding something specific and very fixable. Iron deficiency is the most common nutritional deficiency in the world, and it disproportionately affects women. According to the World Health Organization, iron deficiency is the most common nutritional cause of anemia, and roughly 30% of women aged 15 to 49 and 37% of pregnant women are affected by anemia globally. And that only counts the women whose iron has already dropped far enough to show up as anemia. Many more are running low on iron stores well before that point, feeling every bit of it, and never getting a simple blood test that would explain why.

The symptoms that get written off

Iron does a lot more than carry oxygen in your blood. It is central to energy production inside your cells, which is why running low tends to show up first as a general sense of being depleted rather than anything dramatic. The classic tip-offs include:

  • Fatigue that rest doesn't resolve — the hallmark symptom, and the easiest to blame on everything else.
  • Brain fog and poor concentration — iron is required for the mitochondrial machinery that powers metabolically demanding tissue, including the brain.
  • Cold hands and feet — reduced oxygen delivery to your extremities.
  • Hair shedding — more strands than usual coming out when you wash or brush.
  • Restless legs — that crawling, need-to-move sensation in your legs at night.
  • Cravings for ice or other non-food items — a compulsion to crunch ice (called pagophagia) is one of the more specific clues, which we will come back to.

Any one of these is easy to explain away. Stacked together, they are worth a closer look — because none of them require full-blown anemia to appear.

Low ferritin vs. anemia: you can be depleted before you're anemic

Here is the piece most people miss. Your body stores iron in a protein called ferritin, and it draws down those stores long before your hemoglobin (the oxygen-carrying protein measured in a standard blood count) drops enough to be labeled anemic. So you can have a perfectly normal complete blood count and still be genuinely iron-deficient, with the symptoms to match. Doctors call this iron deficiency without anemia, and it is common precisely because the routine test most people get doesn't measure iron stores at all.

This isn't a fringe idea. In a randomized controlled trial published in CMAJ, researchers recruited 198 menstruating women who complained of fatigue and had low iron stores (ferritin below 50 µg/L) but normal hemoglobin — meaning, by the usual definition, they were not anemic. Half received oral iron for 12 weeks and half received a placebo. The iron group's fatigue score dropped by nearly 48%, compared with about 29% in the placebo group. These were women who, on a standard workup, would have been told their blood was fine.

The ice-craving clue fits the same pattern. In a study of blood donors, 11% of those who were iron-depleted or deficient reported pica (cravings for non-food substances like ice), compared with just 4% of iron-replete donors. Restless legs showed up too, though less strongly (16% versus 11%). The researchers concluded that a craving to crunch ice is a genuinely useful flag for low iron — so if that habit sounds familiar, it is worth mentioning to your doctor rather than laughing off.

Why "normal" on your lab results might not mean fine

Even when ferritin does get measured, the interpretation trips people up. Lab reference ranges are built from population averages, not from the level at which you personally feel well. A review of iron deficiency without anemia notes that while the WHO uses a ferritin below 15 µg/L to diagnose iron deficiency, in everyday clinical practice a value under 30 µg/L is widely used to identify it — and many labs flag a result as "normal" well below where symptoms can begin. The review is blunt that reference ranges are averages and should not be treated as hard cutoffs.

In practice, that means a ferritin of 18 or 25 might land inside the "normal" column on your printout while your body is clearly telling you otherwise. The trial above deliberately enrolled women with ferritin under 50, and they still improved with iron. If you have persistent symptoms and a low-but-"normal" ferritin, that is a conversation to have with your doctor, not a closed case.

Who is at highest risk

Iron deficiency doesn't hit everyone equally. The groups most likely to run low include:

  • Menstruating women. Monthly blood loss is a steady drain on iron, which is exactly why the recommended intake for premenopausal women is 18 mg a day, versus just 8 mg for men and for women past menopause. Heavy periods raise the risk further.
  • Pregnant women. Blood volume expands dramatically and the growing baby draws on the mother's stores, which is why iron needs jump during pregnancy.
  • Endurance runners and other endurance athletes. Distance running in particular chips away at iron through several routes at once. A scoping review of long-distance runners describes foot-strike hemolysis (repeated impact damaging red blood cells), small amounts of gut bleeding, iron lost in sweat, and exercise-driven inflammation that raises a hormone called hepcidin, which temporarily blocks iron absorption. For a woman who runs, those losses stack on top of monthly ones.
  • Frequent blood donors. Each whole-blood donation removes a meaningful amount of iron, and regular donors can gradually deplete their stores — as the donor study above showed.
  • People eating mostly plant-based. Not because plants lack iron, but because the form of iron in plants is absorbed less efficiently, as we will get into next.

Heme vs. non-heme iron, and the vitamin C trick

Dietary iron comes in two forms, and they behave very differently in your gut.

Heme iron comes only from animal foods — red meat, poultry, and seafood. It is absorbed efficiently and fairly reliably. Non-heme iron comes from plants (beans, lentils, tofu, whole grains, nuts, seeds, leafy greens) as well as eggs and fortified foods. It is more abundant in most diets but absorbed far less efficiently, and its uptake is strongly influenced by what you eat alongside it. According to StatPearls' overview of dietary iron, roughly 25% of heme iron is absorbed versus 17% or less of non-heme iron, and overall iron bioavailability runs about 14 to 18% on a diet containing animal products but as low as 5 to 12% on a plant-based one.

That gap is exactly why the pairing tricks matter so much, especially if you eat little or no meat:

  • Add vitamin C to iron-rich plant meals. Vitamin C converts non-heme iron into a form your gut absorbs more readily and keeps it soluble. Even a small amount — the vitamin C in a squeeze of lemon, some bell pepper, tomatoes, strawberries, or citrus — can meaningfully boost absorption from the same meal.
  • A little meat, fish, or poultry helps the plants, too. The same review notes that adding chicken, beef, or fish to a meal can increase non-heme iron absorption two- to threefold.
  • Watch the inhibitors at mealtime. The polyphenols in tea, coffee, and red wine, plus calcium (from dairy or supplements) and phytates in some grains and legumes, all blunt iron absorption when consumed with the meal. You don't have to give any of them up — just move that cup of tea or coffee, and your calcium supplement, to between meals rather than alongside your iron-rich plate.

In other words, a lentil stew eaten with a glass of milk and finished with a cup of tea is working against you. The same lentils cooked with tomatoes and peppers, with the tea saved for an hour later, are working for you.

A realistic food-first plan

You don't need organ meats and a supplement regimen to get more iron. You need a handful of iron-rich meals you will actually make, built with absorption in mind. A few examples that pair iron with vitamin C or heme sources:

  • Lentil and tomato stew with red bell peppers, finished with a squeeze of lemon.
  • A spinach salad with pumpkin seeds, chickpeas, and orange or strawberry slices.
  • Beef or chicken stir-fry loaded with broccoli and peppers.
  • Fortified oatmeal topped with berries (save the coffee for mid-morning).
  • Sardines or canned salmon on whole-grain toast with tomato.
  • Tofu and broccoli with a citrus-based sauce over rice.
  • White bean and kale soup with a tomato base.

The pattern is simple: build the meal around an iron source, invite vitamin C to the table, and keep the big inhibitors (tea, coffee, a calcium supplement) for a different part of the day. Spreading iron-rich foods across the week matters more than any single "super meal," because absorption is limited per sitting.

When to test, and when to leave it to a doctor

Food-first is the right foundation, but iron is one nutrient where you should not simply start throwing supplements at the problem. Here is the responsible path:

Get tested rather than guessing. Fatigue has many causes, and the only way to know if iron is one of yours is a blood test. Ask your doctor specifically for a ferritin level (and ideally a full iron panel), not just a standard blood count — remember that a normal complete blood count can miss depleted stores entirely. If your ferritin is low or low-normal and you have symptoms, that is the signal to act, ideally with your doctor interpreting the result in the context of how you actually feel.

Don't self-prescribe iron pills. More is emphatically not better with iron. Your body has no efficient way to get rid of excess, and it can build up to harmful levels. The tolerable upper limit for adults is 45 mg a day, and high-dose supplements commonly cause nausea, cramping, and constipation well below toxic levels. More seriously, some people carry a genetic condition called hemochromatosis that causes them to absorb too much iron; unnecessary supplementation can accelerate damage to the liver, heart, and other organs. That is why iron supplements belong on a doctor's recommendation and, when used, a doctor's follow-up — not a self-directed guess based on a blog post.

"I'm just tired" is one of the most under-investigated sentences in women's health. Iron deficiency is common, it disproportionately affects menstruating women, pregnant women, runners, and blood donors, and it can quietly sap your energy long before it ever shows up as anemia on a basic blood test.
